When 42-year-old Ms. Meng held a checklist with an AMH of only 0.5, many hospitals ruled that her "natural pregnancy probability was less than 1%." Reproductive medicine research has shown that the success rate of test tube is closely related to the woman's age, ovarian function and uterine status. Clinical practice suggests that meeting three key "A"-age (Age), anti-Mullerian hormone (AMH), and endometrial shape (type A endometrium) can significantly increase the probability of pregnancy and lay a scientific foundation for test-tube therapy.
1. Age: The "golden window" for fertility
Ageis the primary natural factor affecting the success of test tubes.
Age is the primary natural factor that affects the success of in vitro fertilization procedures.
Ideal age: Women 25-35 years old are suitable for test tubes, with strong ovarian function and high eggquality
Challenge of advanced age: After the age of 35, the decline of ovarian reserve accelerates, and the abortion rate and risk of pregnancy complications (such as hypertension and diabetes) increase significantly; those over the age of 45 further drop to about 2%.
Age-related challenges: After the age of 35, ovarian reserve declines rapidly, and the risk of miscarriage as well as pregnancy complications such as hypertension and diabetes increases significantly. For individuals over 45 years old, this risk further decreases to around 2%.
Male sperm quality declines with age, and the risk of chromosomal abnormalities increases, which may affect embryo health, but the effect is weaker than that of women.
The quality of male sperm declines with age, and the risk of chromosomal abnormalities increases, which may affect the health of embryos. However, this effect is less significant compared to females.

2
. Anti-Mullerian hormone (AMH): a "barometer" of ovarian reserve
AMH directly reflects the inventory of ovarian volunteers and is the core indicator for formulating test tube plans.
Normal range: 2-6.8 ng/mL indicates good ovarian reserve, and the number of oocytes retrieved is 8-15, which can ensure embryo screening space.
The normal range is 2-6.8 ng/mL. A value within this range indicates good ovarian reserve. Ideally, the number of eggs retrieved should be between 8 and 15, which ensures sufficient space for embryo selection.
Critical risk: When AMH is less than 1 ng/mL, it indicates severe decline in ovarian function, and the chances of natural pregnancy are extremely low. Even with assisted reproduction techniques, the number of eggs available per cycle may be only 1-5. However, young patients still have a chance of success.
When AMH is lower than 2 ng/mL, accelerated intervention is needed, such as using a microstimulation regimen or cumulative cycle oocyte retrieval, to make up for the quantity disadvantage.
When AMH levels are below 2 ng/mL, immediate intervention is necessary. Strategies such as micro-stimulation therapy or extended egg retrieval cycles can be used to compensate for the reduced number of eggs available.

3
. Endometrium (Type A): The fertile ground for embryo implantation
The receptivity of the endometrium determines whether the embryo can be successfully implanted.
Advantages of Type A: Under ultrasound, it shows a clear "three-line sign"(strong echo in the outer layer, hypoecho in the inner layer, and obvious midline), with a thickness of 7-14mm, and rich blood flow, providing a suitable implantation environment for the embryo.
Type A characteristics: Under ultrasound examination, it exhibits a distinct “three-line pattern” – with a strong echo layer on the outer surface, a low-echo layer in the middle, and a clear midline. The thickness ranges from 7 to 14 mm. It also has abundant blood flow, providing an ideal environment for embryo implantation.
Type B/Limited: Type B (three-line sign blurred), Type C (homogeneous strong echo). Due to structural or blood flow abnormalities, the resistance to implantation increases.
Intimal polyps, inflammation, or uneven thickness require early hysteroscopic treatment. Combined with hormone regulation (such as estrogen support) can optimize the endometrial morphology.
Endometrial polyps, inflammation, or uneven thickness of the endometrium require early treatment through hysteroscopy. Combining this treatment with hormonal regulation (such as estrogen support) can improve the shape of the endometrium.
③ Early intervention and earlytreatment
Start the test-tube cycle before the age of 35, and combine AMH testing to evaluate ovarian reserve to avoid delaying the timing of childbirth.
Start a in vitro fertilization cycle before the age of 35. Combine this with AMH testing to assess ovarian reserve and avoid delaying the timing of childbearing.
Even if the AMH is low or the age is older, high-quality embryos are selected through Embryo Genetic Screening (PGT) and the endometrium is adjusted to TypeA
④ Lifestyle and PsychologicalSupport
Smoking cessation and alcohol restriction, balanced nutrition and stress-reducing conditioning can help improve egg quality and uterine blood flow.
Quitting smoking and limiting alcohol consumption, along with maintaining a balanced diet and managing stress effectively, can help improve the quality of eggs and blood flow in the uterus.
The three "A's" are not isolated indicators, but interrelated fertility assessment systems.
“A”,.Reproductive experts suggest that couples should comprehensively assess their age, AMH and endometrial status before the test tube, and formulate personalized plans, so that they can avoid detours on the test tube road and welcome the arrival of healthy new life.
