Progesteroneis a key reproductive hormone, and fluctuations in its level are directly related to pregnancy outcome. Recent research has confirmed that abnormal increases in progesterone in the early stages of menstruation and before oocyte retrieval may delay the pregnancy window, reduce blastocyst availability, and even lead to a decrease in embryo development potential. This finding provides an important intervention target for patients with repeated implant failure and unexplained infertility.
1. The dual fertility risk mechanism of excessive progesterone
1. Early menstruation: postpone the pregnancywindow
Clinical phenomenon: No increase in cervical mucus (increased leucorrhea) before ovulation suggests a delayed pregnancy window.
Clinical phenomenon: The absence of increased cervical mucus before ovulation indicates a delayed conception window.
Core mechanism: During the follicular phase, when progesterone levels are high and estrogen levels are low, this can inhibit the secretion of cervical mucus and the synchronization of the endometrium. As a result, the penetration rate of sperm and the implantation rate of embryos decrease.
2. Test tube trigger day: damage to embryo quality
Key study: A retrospective analysis of 1109 test tube cycles showed that progesterone levels before egg retrieval were negatively correlated with the proportion of available blastocysts on day5
Progesterone <0.5ng/ml → Available blastocysts accounted for 68.5%
Progesterone>1.15ng/ml → The proportion of available blastocysts dropped sharply to 52.3%(a decrease of more than 10%).
Progesterone levels exceed 1.15 ng/ml. The proportion of available blastocysts drops significantly to 52.3%, representing a decrease of more than 10%.
Pathological basis: High levels of progesterone inhibit the differentiation of follicular granulosa cells, resulting in insufficient energy reserves for the egg, delayed maturation, or even cessation of development.
1. Etiological traceability andmonitoring
Incomplete degeneration of corpus luteum: The main cause of high progesterone in the early stages of menstruation (corpus luteum did not completely transform into white body in the previous cycle), and ovarian blood supply disorders or chronic inflammation need to be checked.
Incomplete regression of the corpus luteum: This is a common cause of elevated progesterone levels in the early stages of menstruation. It usually occurs when the corpus luteum does not fully transform into the corpus album during the previous cycle. It is necessary to investigate possible obstructions in ovarian blood supply or chronic inflammation as potential causes.
Trigger day warning threshold: If progesterone levels exceed 0.79 ng/ml, it will significantly affect the development of blastocysts. It is necessary to continuously monitor LH levels and prolactin levels.
Drugregulation
Patients with luteal dysfunction: Supplementing exogenous progesterone to improve ovarianmicrocirculation
Hyperprolactinemia: Use of dopamine receptor agonists (such as bromocriptine).
Hyperprolactinemia: Dopamine receptor agonists such as bromocriptine can be used for treatment.
When progesterone>1.5ng/ml on the trigger day, cancel fresh embryo transfer and switch to whole embryo freezing + freeze-thaw cycle transfer (FET)
3. Lifestyleintervention
Reduce staying up late and stress (reduce the inhibition of cortisol on the gonadal axis)
Avoid high-fat diets (fatty acid metabolism interferes with hormone balance).
Avoid high-fat diets (fatty acid metabolism can disrupt hormonal balance).
1. Precise control of windowperiod
For those with high progesterone in the early stages of menstruation, short-term increase in the estrogen/progesterone ratio by estrogen patches can restore the normal pregnancy window to 80% of patients.
For patients with high levels of progesterone during the early stages of menstruation, using estrogen patches to temporarily increase the ratio of estrogen to progesterone can help 80% of patients regain normal fertility.
Time-lapse monitoring: dynamically observe the division rate of embryos in the high progesterone cycle, and screen for transplantation with stronger developmentpotential
Assisted activation technology: Calcium ionophore activation (ICSI-AOA) is used to improve fertilization rate for premature oocytes caused by granulosa cell dysfunction.
Auxiliary activation technology: For cases where premature ovulation occurs due to dysfunction of granulosa cells, calcium ion carriers are used for activation (ICSI-AOA) to improve the fertilization rate.
:“”,.If people planning for pregnancy have repeated miscarriages or failed transplantation, they need to simultaneously screen for thyroid function, anti-phospholipid antibodies and endometrial receptivity.
Optimization of fertility needs to penetrate the appearance of hormone fluctuations and capture key pathological links.
,.From progesterone monitoring to the reconstruction of corpus luteal function, from embryo freezing to endometrial synchronization correction-the essence of scientific pregnancy preparation is to use precision medical treatment as a blade to break the maze of difficult pregnancy.
